Jim and I played the longest game in league history on May 30 - a 19-inning affair that finally ended when a tired Johan Santana gave up a game-winning single to Rafael Furcal. Santana came in to pitch in the 16th inning, when Wellwood's bullpen was empty. It led to some confusion, trying to figure out who should pitch and for how long. I'm making this a sticky so you have something to refer to if you find yourself in a similar situation.
1) When you get down to your last reliever, you have to pitch him the maximum amount of innings he can throw before replacing him. 2) Once the bullpen is completely empty, the manager has the option of bringing in the next day's starter OR the pitcher who started two games before. The pitcher who started the previous game IS NOT eligible to pitch. 3) Use the Starting Pitchers Rest Chart on the blue paper chart (next to the Super Advanced Injury Chart) to determine if either starter can come into the game fully rested or if they will be pitching tired. Refer to the amount of innings each starter threw in the previous outing to make that determination. 4) The pitcher you choose will be able to pitch the amount of innings it shows on his card. If he is pitching on full rest, the normal rules for making him tired will apply. If he is pitching tired, he will be tired for the entire appearance. 5) The pitcher you choose will NOT be able to make his next scheduled start. He will need to rest a minimum of three games after the relief appearance. Teams using starting pitchers in relief will need to make a roster move to recall a pitcher to make an emergency start.

This is a good start. I would add/clarify two items...
1. Reminder - You can also use a position player to pitch using the appropriate chart in the rulebook (ask me if you would like a copy). 2. We should clarify the rules as they relate to the playoffs where roster moves are not permitted. This happened to JB and I a few years back in th World Series where he was forced to bring in starters in relief and then needed to start them tired because he didn't bring an extra starter.
